PCSO.

office of Mabel Mamba PCSO Director and was given a guarantee letter for his
financial assistance.

Apuado is from san Pedro, Vigan City, Ilocos Sur. She was operated last July 21,
2011 with cancer of the ovary at the Manila Adventist Medical Center, Manila.
JCS Foundation assisted her and she was given a guarantee letter for her
hospitalization and Radio therapy respectively by the PCSO.

year old John Lorenz Arizala of Camanggaan, Vigan City, Ilocos Sur expired after
suffering from Cardiorespiaratory Arrest,
Dengue Fever Grade IV, Severe Metabolic Acidosis probably secondary to Inborn Error of
Metabolism, t/c Mitochondrial Disorders, Epilepsy, GTC probably cryptogenic and
r/o Sepsis. He was granted a guarantee letter from PCSO through the
endorsement of JCS Foundation last September 20, 2011.

Merly Galacan of Brgy. VII, Vigan City, Ilocos Sur, 47 years old suffered from Pulmonary Edema Secondary to HCVD, IF, II-B,
Hypertension, essential Gr.II Ischemic Heart Disease, Unstable Angina, UGIB,
Secondary to Stress Ulcer, CAP High risk, BAIAE. She was given a guarantee letter from PCSO through the effort of JCS foundation.

Arcena of Camanggaan, Vigan City, Ilocos Sur. She was also assisted by JCS
Foundation for the immediate release of her guarantee letter from PCSO.

Uday of Mindoro, Vigan City, Ilocos sur. She was assisted by JCS Foundation and
was given a guarantee letter from PCSO.

Divina 9 year old boy is from Nalvo, Sta. Maria, Ilocos Sur undergone operation
from fracture on his left arm and he was endorsed to PCSO by JCS Foundation. He
was given a guarantee letter from PCSO.

Allan Itchon from Vigan City, Ilocos Sur was hospitalized at St. James hospital
due to asthma and weak lungs. He was also a recipient of a guarantee letter from
PCSO through the effort of JCS Foundation.
